TL;DR: patch the VPN, then treat every password it has seen as exposed
Attackers chained three Ivanti Connect Secure flaws to get root on VPN appliances, starting no later than 3 October 2026, on a box that was fully patched at the time. Once inside, they collected domain credentials, searched Active Directory for local admin passwords and BitLocker recovery keys, sniffed network logins and cleaned the logs. Ivanti fixed the last link of the chain on 29 October, and CISA listed it as exploited on 30 October. If you run Ivanti Connect Secure, update it, follow Ivanti's recovery steps, reset the passwords of every account the appliance could see, and check who opened your PDM vault and file shares since early October. Keep your own dated proof of your key designs, so a breach does not also become a fight about who designed what.
